Photo ID: Please make sure you bring your photo ID with you to every appointment. If you are a CDL holder or coming for a DOT exam or drug screen, this MUST be your driver’s license.
Authorization Form: If you are being sent by your employer, please bring your authorization form, ESPECIALLY if this is a drug screen or for lab work. This should have been provided by your employer.
Company-Specific Paperwork: If you have forms that you need filled out for your employer, please bring these with you.

Booking an appointment with us is simple: visit our “Book an Appointment” section on the website or calling our office. Our team is ready to assist and schedule a suitable time for you. In most cases, we can get you in same day or next day!

Service agreements are NOT required to receive care at Geer Care. However, we do encourage companies to establish service agreements in order for us to streamline care by identifying what services are needed and if any forms need to be completed.

Yes, both of our nurse practitioners are FMCSA certified and on the National Registry of Certified Examiners list. In addition, our medical assistant is a DOT certified collector.

While appointments are generally required, we do understand urgent needs. We can almost always accept you same day or next day. Calling ahead can streamline your experience and potentially reduce your wait.

Yes! We will come to you to perform any necessary examinations. All of our equipment is portable, and we will travel to you. We can cater to the needs of your organization and can work around your needs!

We are stern followers of the Health Insurance Portability and Accountability Act (HIPAA). We do not transmit results anywhere without your direct permission. However, with your permission, we can send a summary to your doctor(s).
